Hospital pharmacy

What is hospital pharmacy?

Hospital pharmacy is a pharmaceutical speciality that serves the population’s pharmaceutical needs, through the selection, preparation, acquisition, control, dispensation and by providing information on medicines and other activities aimed at achieving an appropriate and safe use of medicines and medical devices, for the benefit of patients treated in the hospital and in its sphere of influence.

Its aim is to contribute to improving patient health with pharmacotherapeutic dispensation and/or follow-up, providing specialised pharmaceutical care that adds value to the care process and promotes and ensures the effective, safe and efficient use of medicines as part of comprehensive and continuous care.

Patients

Hospitalised patients: the hospital pharmacist is involved in the clinical management of the patient’s pharmacotherapy, guaranteeing safe and efficient use of medication during admission and discharge.

  • Outpatients: provides patients with medications indicated and prescribed by a doctor of the centre that need special follow-up due to their characteristics and therefore are only dispensed at the hospital pharmacy.
  • Onco-haematological patients: the pharmacy service validates (review and confirmation by the pharmacist) and prepares the prescribed treatment, adapting it to the centre’s current protocols and the patient’s clinical characteristics. Pharmaceutical care is also provided.
